No.4 sight blade chart?

Does anyone have access to the chart that helps determine what sight blade height you need? I had it printed out but lost it. I just got another new No.4 which I haven't shot it but just in case the POA VS. POI is incorrect.

that chart came in handy a few times. I once had a No.4 that was 19 " high at 100, I have no idea how the previous owner hit a damned thing with the sights off that bad. I ended up having to build the blade up with JB weld......that sucked.

Each step in change in sight blade =
 
1.87" @ 100 yds
0.475" @ 25 yds

My newest acquisition does have a blade but my ammo hasn't been delivered yet so I can't shoot her to find out whether my POA and POI is correct.
