Just a thought... is the thick "washer" between the "dogbone" and the safety parts lined up? I believe the "dogbone" part that screws into the wrist band has a V notch and the washer has a raised V that need to match up. (I may have my V's mixed up. I just took my first No1 safety apart the other day!".

Also make sure its the right way round, one side is "ridged". If that doesn't fix it the spring may be weak or bent. If so gently bending it a little may be a cure. Don't over do it though they can be snapped. Check the position of the safety catch (little comma shaped dingus on the safety lever's multi-start thread.)
There's a "one thread off" position that will give you that effect. screwed IN fully this should be the relationship.

I do think I was one thread off. I’ll know for sure next time I shoot. It also feels like the notch and recess on the safety never clicks in (doesn't seem to line up). feels more like tension holding it in place, we’ll see. Thanks for pointing out the threads and hopefully that solved it.
