Preface: "I had no other option than to do what I did."

I found the No4Mk1* 23L8*** barreled action in a 55 gallon drum with a bunch of other rifle parts. It was missing both axis pin holes/ears for the rear sight assembly. The owner of the gun store sold it to me for $20.00. As soon as I returned home I got busy with my Dremel Tool and a bunch of cutoff wheels. Drilled and tapped the holes for the PH scope base, trimmed the barrel back to 22.5" and then polished everything that I could. I used Van's Instant Gun Blue for the metal finish. The original bolt was refitted and headspace was well within tolerances with the #0 bolthead supplied. The stock is the ATI zytel but has been camo dipped. The scope is a Bushnell Prime 6-18×50 with side focus and is cradled in Bisley T050 1" scope rings.  It is what it is folks...
Post Script: the 5 groove barrel has two small circles with numbers inside them stamped near the knox form indicative of Maltby manufacturing.

The angled bases are not rare. It is getting harder to find the dogbone 17mm mount however...

I suspect they are tricky to mount in regards to making sure you get a level platform for the rings/scope.

Getting the three holes in alignment is crucial but if you come up short you can add a tiny forward cant by adding tiny relief cuts on the outer screw holes on the scope base. It's not too noticeable in this photo but, the back of the scope base is 3 thousandths of an inch higher than in the front...

It should be noted here that, as of yesterday afternoon, I am not the owner of this rifle any longer. I was showing it to the gun store owner that I bought it from and was comparing it to a complete No4Mk1* when a gentleman offered me $500.00 dollars for it on the spot. The transaction was taken outside and completed there...
